COACHES - please see the following information:
ALL COACHES MUST OBTAIN A COJ PARKS AND RECREATION BADGE EACH YEAR. Badges must display the current year and all badges expire Dec 31 of the year listed on the badge. Even if you have obtained a badge for the first time during a fall season, your badge still needs to be renewed for the new calendar year. For example, if you received a 2022 badge in FALL 2022, it expired on Dec 31, 2022 and you must complete a Continuous Service form as of Jan 1, 2023 in order to obtain a valid 2023 badge.
Steps to follow:If you are NEW to coaching and/or you DO NOT have a badge with last year listed (2022) - please download and complete the form below titled "New Background Check Consent Form (PDF)" and call Mike James with COJ to schedule a *NEW* background check appointment. The phone number is 904-255-7925. You will need to complete the appointment in-person at 214 N. Hogan Street, Jacksonville, FL 32202 (1st floor, door to the right).
If you are a RETURNING coach with a current badge that has last year listed (2022) - please download and complete the form below titled "Continuous Service Form (PDF)" and email this to the park -
[email protected]. These must be completed in full annually, and will be sent to Mike James with COJ. Once your form is processed by COJ Parks and Rec, a new badge will be issued and distributed to you.
