COACHES - please see the following information:
ALL COACHES MUST OBTAIN A COJ PARKS AND RECREATION BADGE EACH YEAR. Badges must display the current year and all badges expire Dec 31 of the year listed on the badge. Even if you have obtained a badge for the first time during a fall season, your badge still needs to be renewed for the new calendar year. For example, if you received a 2022 badge in FALL 2022, it expired on Dec 31, 2022 and you must complete a Continuous Service form as of Jan 1, 2023 in order to obtain a valid 2023 badge.
Steps to follow:If you are NEW to coaching and/or you DO NOT have a badge with last year listed - please download and complete the form below titled "New Background Check Consent Form (PDF)." You will need to complete this in full, and email both the form and a headshot photo to Mike James, COJ Parks & Recreation, by email (
[email protected]). If you have questions, his phone number is 904-255-7925. If needed, he will instruct you with further directions.
If you are a RETURNING coach with a current badge that has last year listed - please download and complete the form below titled "Continuous Service Form (PDF)" and email this to Mike James, COJ Parks & Recreation (
[email protected]). These must be completed in full annually in order to be approved and for a current badge to be printed. If you have questions, please contact

Badges will be picked up by a SJAA Board Member and distributed with game balls and uniforms.
