The Head Coach of each team will receive a roster and uniform order form following team formation. -If you are a returning coach, DO NOT use any forms from previous years. -Forms must be completed in the park's shared Google sheet. -If you do not complete all fields correctly, the form will be returned, or you will receive standard selections. Please note: 1) If you want your team name displayed on your jersey, the specific font name/style MUST be indicated by the Coach and included on the form. If you do not indicate a font name/style, a standard font will be used. 3) All team names will be displayed in white font on the sleeve of the tshirt. 4) Coaches are responsible for indicating the jersey color and logo color when you submit your order form. 5) Hat color will be chosen based on Jersey and Logo color selections. Specific hat requests cannot be guaranteed.Jersey Color Choices:Note: All teams will receive a dri-fit t-shirt (not 2 button as pictured).
